Reducing Emergency Hospital Admissions and A&E Attendance

Havoc is a software tool for GP practices that identifies patients who are using, or are more than likely to be using, health resources such as non-elective hospital admissions and attendance at A&E departments.

Using data readily available from your practice database, Havoc enables you to target and manage multi-disciplinary patient reviews.

How it works

Havoc periodically interrogates the data from your SystmOne or Emis practice database and automatically ranks patients based on key events such as

  • Emergency hospital admissions
  • A&E attendance
  • Attendance by ambulance crew
  • Out of hours contacts
  • GP home visits

Using a colour-coded grid Havoc will the immediately identify the highest consumers of resources over a chosen period, and the number of times each event has occured.

In addition to the key events Havoc may be configured to display important dates of interest for each patient such as

  • Date of last multi-disciplinary case review
  • Date patient was added (or removed) from the community matron caseload
  • Date of OOH notification
  • Date of anticipatory care plan
  • Date of admission to a Care Home

Havoc is also a versatile case management tool allowing you view historical events associated with a patient and record notes for new case reviews
